How to Apply for a Copy of a Birth Certificate in New York State

 

To apply for a birth certificate from New York State, the birth must have occurred in New York. You may use the application if you are the person named on birth certificate or are the parent of the person named on the birth certificate.  The requesting parent’s name must be on the birth certificate.  The application can be downloaded from www.health.state.ny.us/vital_records/forms/doh-4380.pdf.

Identification Requirements. You must submit the application with copies of either A or B:

A

Provide One of the following photo-ID

Þ Driver license

Þ Non-driver photo ID

Þ Passport

Þ Other government issued-ID 

Or, B

Two of the following showing the applicant’s name and address:

Þ Utility or telephone bill

Þ Letter from a government agency dated within the last six months.

 

The cost to receive a copy of your birth certificate is $30.00 per copy.
